Adult Day Health Care Waiver (ADHC)

The Adult Day Health Care Waiver (ADHC) provides certain services for a portion of the day in a licensed and Medicaid enrolled facility, and services in the home to individuals who qualify. This can often be an alternative to nursing home care for people who do not need care 24 hours, 7 days per week.

Each of Coastal Care’s certified Support Coordinators will assist families with the following:

  • Completing accurate assessments that are thorough and comprehensive
  • Completing person-centered Comprehensive Plan of Cares that are accurate and complete
  • Coordinating and monitoring services within clients allowable budgets
  • Making recipients aware of their rights and ensuring rights are protected
  • Coordinating formal and informal resources that are needed and appropriate
  • Encouraging problem solving with providers when issues arise
  • Being available 24 hours per day for recipient crisis

Each recipient of Medicaid waiver and targeted support coordination has the freedom to choose their provider. The individual’s choice of support coordination or direct service provider does not affect their ability for waiver and state-plan programs or support coordination services.
