The Adult Day Health Care Waiver (ADHC) provides certain services for a portion of the day in a licensed and Medicaid-enrolled facility, and services in the home to individuals who qualify. This can often be a home and community-based alternative to nursing home care for people who do not need care 24 hours, 7 days per week.
The Community Choices Waiver (CCW) serves seniors and persons with adult-onset disabilities as an alternative to nursing facility care. The CCW contains a wide array of services including support coordination, nursing and skilled therapy assessments and services, in-home monitoring systems, home modifications and assistive technologies, personal care, home-delivered meals, and caregiver respite.
EarlySteps Early Intervention, a state and federally-funded program, provides services to eligible families with infants and toddlers aged birth to three years (36 months) who have a medical condition likely to result in a developmental delay, or who have developmental delays. Children with delays in cognitive, motor, vision, hearing, communication, social-emotional, or adaptive development may be eligible for services.
