The Adult Day Health Care Waiver (ADHC) provides certain services for a portion of the day in a licensed and Medicaid-enrolled facility, and services in the home to individuals who qualify. This can often be a home and community-based alternative to nursing home care for people who do not need care 24 hours, 7 days per week.
The Community Choices Waiver (CCW) serves seniors and persons with adult-onset disabilities as an alternative to nursing facility care.
EarlySteps, a state and federally-funded program, provides services to eligible families with infants and toddlers aged birth to three years (36 months) who have a medical condition likely to result in a developmental delay, or who have developmental delays.
